Islington Health Outreach

The Islington Health Outreach Team is a primary care health service that delivers holistic healthcare to people experiencing homelessness in Islington.

We provide monthly hub-based drop-in clinics at Solidarity Hub on Seven Sisters Road and The Manna Project at St Stephen’s, Canonbury. Our clinical team also provide clinical care and advice during roaming sessions at various hostels in Islington.

Our goal is to provide high quality medical care including baseline holistic health checks and care planning with a strong preventative approach. A central theme of our work is to achieve as much proactive care as possible at the first meeting with the client.

The service aims to raise awareness and understanding of this client groups unique needs. We have delivered presentations and made educational contributions at various forums including the Homeless Prevention and Rough Sleeping Forum, the Homeless Health and Care Practitioner Network and the London Network of Nurses and Midwives Conference.

Our clinical team is made up of two GPs and three outreach nurses. The team is led by Clinical Lead, Dr Polly Wootton and Service Lead, Jo Williamson

Our community based clinics for people experiencing homelessness are drop in access and no appointment or referral is needed. Both hub clinics have access to shower facilities:
