Quality Improvement

Our Quality Improvement Support Team (QIST) is a multi-skilled group capable of interrogating practice data and developing bespoke interventions to improve patient wellbeing. The team brings together specialist data analysis with clinicians and prescribers to focus right down to individual patient level.

Its overarching goals are to reduce unwarranted variation within the health economy and support practices in delivering the NHS “Triple Aim” of health and wellbeing outcomes, patient experience outcomes and efficiency outcomes.

The team provides hands on practical help for practices and Primary Care Networks to develop:-

  • Consistent standards and service to all patients

  • Innovative and new ways of working

  • Rigorous and systematic approaches to patient care

  • An increased focus on prevention and self-management

The first major subject tackled was Diabetes in which the “D-QIST” team focused specifically on improving the three key clinical indicators of HBa1C, Cholesterol and Blood Pressure.

Since then, the team has undertaken many projects focusing on things like COPD, hypertension, moderate frailty, serious mental illness and learning disabilities.
